Join Our EPD Smile Dental Plan
If you don’t have dental insurance, be sure to ask our staff for details on our EPD Smile Dental Plan. For the annual enrollment cost of $450, your child will receive:
- Two dental cleanings
- Two dental exams
- X-rays as needed
- 15 percent off restorative dental treatment
- 10 percent off preventive dental treatment
Unlike dental insurance, there are:
- No monthly premiums
- No deductibles
- No enrollment forms
- No maximum allowed benefits
- No claims to fill out for reimbursement

Dr. Susan and our staff keep it simple for new patients coming to our office. One of the ways we do this is by making our new patient forms available online.
That’s one less thing you’ll have to worry about when you come for your first visit. We don’t want you having to juggle your driver’s license, insurance card, and other belongings while filling out paperwork. Just fill out these forms ahead of time at your convenience, and you’ll be set!
In addition to our easy online forms, we want to make paying for your dental care as simple as possible too. We accept many payment options, including:

We also accept several dental insurance plans including Delta Dental.
We’ll be happy to file the claim for you and accept assignment of benefits so you won’t have to fill out any insurance paperwork.
